About the Therapist

Hazel Pracht, PT, C/NDT

Hazel is the owner of Rising Care Rehab and a licensed physical therapist with more than 18 years of clinical experience. She founded the practice to bring specialized, unhurried pediatric care into the place where children learn best — their own home.

Hazel is certified in Neuro-Developmental Treatment (NDT), an advanced, evidence-based approach to working with children who have motor delays, neurological differences, and complex movement needs. Her sessions blend hands-on technique with play-based engagement so children stay confident and parents stay informed.

Outside of work, Hazel is an active volunteer at her church and enjoys reading, cooking, traveling, and spending time with her husband and two children.
